2nd VISIT REMINDER FOR PARENTS OF YEAR 8 STUDENTS

- 2ND DOSE HUMAN PAPILLOMAVIRUS (HPV) (GARDASIL 9)
SCHOOL IMMUNISATION PROGRAM
Year 8, 2nd Visit
For Human Papillomavirus Vaccine (HPV)
Eastern Health Authority (EHA – www.eha.sa.gov.au) will soon visit St Peter’s College to administer the 2nd dose of HPV (Human Papillomavirus (Gardasil 9)) vaccination for those students with parent/guardian consent.
If you did not receive an immunisation consent form from your child, please ask at the school office for one or if unavailable contact EHA on 8132 3600.
If your child was absent for the 1st visit, please visit www.eha.sa.gov.au to make an appointment or call 8132 3600 to advise if the missed vaccine was administered at another provider (eg, GP). We will only be bringing the above vaccine to the school for the 2nd visit.
If any information provided on the original consent form has changed, please inform EHA immunisation staff before the date of vaccination on 8132 3600.
Please notify us if:
- Your child has already started the above program with another provider
- You wish to withdraw your consent
- Your child’s medical condition has changed
Remember: It is your responsibility to notify EHA of any change of information, particularly in relation to your child’s medical condition, on the day of immunisation or prior. A signed and dated note in your child’s diary from a parent or guardian presented on the day is acceptable.
